I'm considering buying an instrument with a missing spike. Is it as simple as (making sure there's not a broken remnant in there) and then just tapping in a new one? Wondering if a new hole is likely needed as the existing one is going to be a little too large or won't hold a new spike.

If there's no spike broken off in there (bright flashlight and perhaps a straight pin can help you determine that) then a sliver or two of flat toothpick  with a small drop of wood glue should reduce the hole nicely and create the friction to hold a new spike in place.

A sliver of ebony or rosewood to match the fretboard might be ideal. But ideal isn't necessary to do the job. I did just as I described almost 15 years ago when my seventh fret spike popped out after nearly 40 years.
