The 2 in parenthesis indicates the hammered note. For example, fret and play the third string at 2, hammer but don't play the fourth string at 2. Your example shows fret at the second fret, hammer on the fourth fret of the same string.

By "phantom hammer-on," I assume you asking about an open-string hammer-on. I would indicate it as shown below.
Your second question describes a hammer-on from the 2nd fret to the 4th fret. I generally would use my index finger on the 2nd fret and hammer with my ring finger on the 4th fret. A slide from 2 to 4 could be used as well.
