Sounds like really fast triplets, six notes to a beat instead of four. Amazing stuff. I bet he practiced that... :o)

Yes, mostly forward followed by backward, pivoting from one to the other when the thumb plays the fifth string.

If you like Scott Vestal's work, triplets in particular, be sure to check the Bluegrass 95,96,97 CDs from back in the 90's. Two of my favorites from Bluegrass 95 are Daley's Reel (caution: in B flat, no capo!) and the superb Goodbye Liza Jane in A (capoed). The latter is probably my all-time favorite banjo break.
